Why questionnaire templates matter
Every new patient starts with paperwork: intake details, consent, and a baseline read on how they’re doing. Embodia’s template library gives you a running start on all three, with pre-built questionnaires, outcome measures, intake forms, and consent forms you can send to patients right away or customize to fit your practice.

How templates are organized
Templates fall into three categories: outcome measures, intake forms, and consent forms. Each category groups templates by purpose, so you can find what you need quickly, whether you’re screening a new patient, tracking progress on a validated measure, or collecting consent before an assessment.
New templates are added to the library over time. If you’re on an Embodia Tier 2 or Tier 3 membership, check your account directly for the most current list — it may include templates not yet reflected here.
